Welcome to My Practice Connect. These Terms of Use (“Terms”) govern your access to and use of the My Practice Connect website, platform, systems, and related services. By accessing or using the platform, you agree to comply with and be bound by these Terms.

If you do not agree with these Terms, please do not use the platform.

Platform Purpose

My Practice Connect provides a secure and compliance-focused prescription workflow infrastructure designed to support licensed healthcare providers and provider-pharmacy coordination processes.

The platform is intended to assist with workflow organization, secure communication systems, operational consistency, and prescription coordination support.

My Practice Connect does not provide medical advice, diagnose medical conditions, prescribe medication, or make independent clinical decisions. All clinical decisions remain solely the responsibility of licensed healthcare providers.

Eligibility

By using the platform, you represent and warrant that:

  • You are at least 18 years old;
  • You are legally authorized to use the platform;
  • Any information you provide is accurate and current;
  • You will use the platform in compliance with all applicable laws, regulations, and professional obligations.

Certain platform features may be restricted to licensed healthcare providers or authorized healthcare personnel.

Provider Responsibility

Licensed healthcare providers maintain full and independent responsibility for:

  • Clinical evaluations;
  • Medical judgment and decision-making;
  • Patient treatment decisions;
  • Prescription authorization;
  • Compliance with applicable healthcare laws and professional standards.

My Practice Connect does not interfere with or direct provider clinical judgment.

Prescription Workflow Coordination

The platform supports workflow coordination and operational communication between providers and certified pharmacy partners.

My Practice Connect utilizes DEA-certified eRx workflow infrastructure and works with LegitScript-certified pharmacy partners, including FDA-registered 503A and 503B facilities when applicable.

Platform participation does not guarantee prescription approval, fulfillment, or patient eligibility.

Compliance and Acceptable Use

Users agree not to:

  • Use the platform for unlawful or unauthorized purposes;
  • Misrepresent identity, credentials, or professional licensing;
  • Upload false, misleading, or fraudulent information;
  • Attempt unauthorized access to systems or data;
  • Interfere with platform operations or security;
  • Violate healthcare privacy, security, or compliance obligations.

My Practice Connect reserves the right to suspend or terminate access for any activity believed to violate these Terms or applicable laws.

Privacy and Data Protection

My Practice Connect implements reasonable administrative, technical, and operational safeguards designed to support secure workflow communication and infrastructure standards.

Use of the platform is also governed by our Privacy Policy.

Intellectual Property

All platform content, branding, workflows, systems, designs, text, graphics, and related materials are the property of My Practice Connect, its licensors, and are protected by applicable intellectual property laws.

Users may not reproduce, distribute, modify, or exploit platform materials without prior written permission.

Third-Party Services and Partners

The platform may interact with third-party healthcare technology systems, pharmacies, communication providers, or related infrastructure services.

My Practice Connect is not responsible for:

  • Third-party system availability;
  • External service interruptions;
  • Independent actions of healthcare providers or pharmacy partners;
  • Third-party policies or operational practices.

Disclaimer of Warranties

The platform and related services are provided on an “as is” and “as available” basis.

My Practice Connect makes no guarantees regarding:

  • Continuous platform availability;
  • Error-free operation;
  • Prescription fulfillment outcomes;
  • Clinical results or patient outcomes;
  • Third-party system performance.

To the fullest extent permitted by law, all warranties, express or implied, are disclaimed.

Limitation of Liability

To the maximum extent permitted by law, My Practice Connect shall not be liable for any indirect, incidental, consequential, special, or punitive damages arising from:

  • Platform use or inability to use the platform;
  • Workflow interruptions;
  • Third-party system failures;
  • Clinical decisions made by healthcare providers;
  • Delays in communication or coordination.

Users assume responsibility for their use of the platform and related operational decisions.

Indemnification

You agree to indemnify and hold harmless My Practice Connect, its affiliates, officers, employees, contractors, and partners from claims, liabilities, damages, or expenses arising from:

  • Your misuse of the platform;
  • Violation of these Terms;
  • Violation of applicable laws or regulations;
  • Infringement of third-party rights.

Modifications to Terms

My Practice Connect reserves the right to modify these Terms at any time. Updated versions will be posted on this page with a revised effective date.

Continued use of the platform following updates constitutes acceptance of the revised Terms.
